• Instructions for downloading and filling out the Publication and Ancillary Study Proposal forms:

  • To fill in the form, first save the file to your computer by right-clicking on the link above, and selecting "Save Target As..."
     

  • Open the file in Adobe Acrobat, click in the first field of the form, and start entering information.  After completing an entry, use the mouse or tab to the next field.  Note that each write-in line is a new field.  Use the tab key to move from the end of one line to the start of the next.  Text will not wrap to the next line.  Also, signature fields cannot be filled in electronically.
     

  • When finished, sign and fax the completed form to Alice Sternberg at the Coordinating Center (Fax:  410-955-0932)
