Dataset Request Tool

General instruction:

Download the Dataset Request Instructions and
the Dataset Request Tool into the root directory (ie, C:\) by right clicking on
"Download SHHS Dataset Request Tool" link below. You will need to do this
in order to enable the help function within the Data Dictionary.

When the Dataset Request Tool is used, it
automatically overwrites itself and will thereafter include selections being made
for variables in the previous data request. DOWNLOAD a blank copy for
each new data request.

Required software & browser: MS
Internet Explorer, MS Access 2000, MS Word 2000. Note: Do
NOT use Netscape or MS Office 97

Before submitting the data request, please
verify that you are using the most current version of the SHHS Dataset Request Tool
(Version 6.4, 28 October 2005) from the SHHS website.
